Your privacy is very important to us and we will ensure what you share is treated as confidential. However, if something is disclosed that has safeguarding implications then it may be necessary to inform the appropriate member of staff and/or relevant agencies.

I have read the above Duty to Inform statement and understand that the Prayer Ministry Team cannot take responsibility for my own actions and /or choices.

I confirm that I have sought this Prayer Ministry, voluntarily, and on my own initiative, and I understand that I am under no obligation to accept any of the ministry given by members of the Prayer Ministry Team.

I understand that the Prayer Ministry Team is made up of volunteers acting on behalf of Trent Vineyard who are not trained or licensed professionals of counselling, therapy or medical services. I agree to release Trent Vineyard Church and its Prayer Ministry Team volunteers from all liability, loss or damage of any kind that may arise from my involvement with this Ministry.
